How does ketamine therapy work?

A different pathway forward.

Ketamine works differently from traditional antidepressants and analgesics. Rather than targeting serotonin, noradrenaline, dopamine or the opioid system, ketamine's main effect comes from targeting the brain’s glutamate system. It does this by blocking a brain receptor called NMDA.

NMDA receptors play a key role in pain perception, mood regulation, and thinking. By calming the NMDA receptor, ketamine therapy can “reset” overactive pain and mood pathways. In doing so it encourages new, healthy nerve cell growth, restoring healthier communication in brain pathways involved in depression and pain.

Intravenous (IV) Infusion

Often considered the "gold standard", intravenous (IV) infusion is the most researched form of ketamine therapy. It delivers ketamine directly into the bloodstream for more precise and reliable dosing. It can be used for treatment-resistant depression, post-traumatic stress disorder and chronic pain.

IV infusion offers a number advantages over other routes. It has tighter control of dosing and rate, allowing adjustments mid-session if needed. It has the highest bioavailability compared to all other routes. And it allows for more flexible dosing protocols.

Intramuscular (IM) and Subcutaneous (SC) Injections

Intramuscular (IM) and subcutaneous (SC) injections provide an alternative for patients who may not require or prefer an IV line. These routes deliver rapid relief and are well-suited to specific treatment needs.

Intranasal Esketamine (Spravato®)

A TGA-approved and now PBS listed option for treatment-resistant depression, esketamine nasal spray is administered under supervision in an authorised centre.

Esketamine nasal spray offers a less-invasive alternative, with rapid onset of effect, though often requiring more frequent maintenance sessions than infusions.

Esketamine nasal spray cannot be used for PTSD or chronic pain, with minimal evidence to support its use in these areas.

Oral therapy

Oral ketamine can be prescribed in select cases, typically as a maintenance strategy following induction. It has less evidence than the other forms of ketamine therapy. It also has lower bioavailability than infusions, injections or nasal spray.

Consultation, screening, and planning

Following referral, we begin with a comprehensive medical and psychiatric assessment, which includes completing screening tests, questionnaires, and a medical consultation. 

This ensures ketamine therapy is safe, appropriate, and tailored to your needs before creating a personalised treatment plan.

During treatment sessions

Once approved for treatment, your sessions can begin. Depending on your treatment plan, this could be in hospital or as a day case at one of our licenced treatment centres.

Your treatment will take place in a calm, supervised clinical environment. You’ll be monitored throughout and vital signs checked regularly, with your comfort and safety as our priorities.

After treatment sessions

Once your session ends, you’ll rest in our recovery area under close nursing supervision. We’ll monitor your vitals, address any side effects, and ensure you feel ready before discharge to your support person. If you are an inpatient in a hospital, you will return to the hospital ward.
